Effective December 2, 2024, you must submit a completed Form I-693 (or partial vaccination record where applicable) together with your Form I-485 Application to Adjust Status otherwise, USCIS may reject your application.

Components of the Exam Include:

  1. Medical History Review-Discussion of past illnesses, surgeries, and medications.
  2. Comprehensive Physical Examination,-Assessment of eyes, ears, nose, throat, heart, lungs, abdomen, skin, lymph nodes, extremities, and vital signs (blood pressure, etc.).

Communicable Disease Screening (per CDC Technical Instructions):

  • Tuberculosis (TB) – Usually QuantiFERON blood test (for ages 2+; chest X-ray if needed).
  • Syphilis – Blood test (RPR or similar; age-specific per CDC).
  • Gonorrhea – Urine test (age-specific per CDC).

3. Vaccination Assessment & Recommendations-Review of records; administration of missing age-appropriate vaccines (e.g., MMR, Tdap, varicella, hepatitis A/B, polio, influenza, meningococcal, pneumococcal). Additional fees apply for vaccines not included.

4. Mental Health & Other Evaluations– Screening for disorders or substance issues that may affect admissibility.

After clearance, the civil surgeon completes and signs your Form I-693 in your presence (do not sign Part 1 until instructed).

You receive the sealed envelope—do not open it—for submission to USCIS ONLY.

Validity of Form I-693 (Important 2025 Update)


A Form I-693 signed by a civil surgeon on or after November 1, 2023, remains valid only while the associated immigration benefit application (e.g., I-485) is pending with USCIS. If the application is withdrawn or denied, the form is no longer valid for future use. USCIS may request a new exam if concerns arise about your health status.
