Forever Young BHRT is a physician-guided medical clinic and anti-aging center located in Oakland Park, Florida. We specialize in Bioidentical Hormone Replacement Therapy (BHRT), Peptide Therapy, Medical Weight Loss, IV Infusion Therapy, NAD+ Therapy, Testosterone Optimization, and Menopause Care, serving patients throughout Broward County and the greater Fort Lauderdale area.
All medical services are provided under the supervision of licensed medical professionals in accordance with applicable Florida and federal law.

Forever Young BHRT provides the following medical and wellness services, subject to clinical evaluation and eligibility as determined by our licensed providers:

All treatment recommendations are made only after a medical evaluation by a licensed provider. Forever Young BHRT reserves the right to decline, modify, or discontinue services based on clinical judgment, patient safety, or medical eligibility.

Booking: Appointments may be scheduled through our website, by phone at (954) 361-6067, or by email at [email protected]. Submitting a booking request does not guarantee an appointment — all appointments must be confirmed by our staff.
Cancellations: We require a minimum of 24 hours' advance notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours' notice may result in a cancellation fee.
No-Shows: Patients who fail to appear for a confirmed appointment without prior notice may be subject to a no-show fee and may be required to provide a deposit for future bookings.
Late Arrivals: Patients arriving more than 10 minutes late may have their appointment shortened or rescheduled at the discretion of our team.
Right to Decline or Reschedule: Forever Young BHRT reserves the right to cancel or reschedule appointments when clinically necessary or when a patient is deemed not an appropriate candidate for the requested service at that time.

Payment is due at the time of service unless prior arrangements have been made with our billing team.

    We accept major credit cards, debit cards, HSA/FSA cards (where applicable), and other approved payment methods.
    Forever Young BHRT is a cash-pay practice. We do not bill insurance for any services. Patients are responsible for understanding their payment obligations before receiving care.
    Prices are subject to change without notice. Pricing discussed during a consultation is an estimate and not a binding guarantee.
    All service sales are final. Refunds are not provided for completed medical treatments or services. If you have a concern about your care, please contact us directly so we may address it.

Treatment packages, prepaid programs, and memberships are subject to the specific terms provided at the time of purchase.
Packages have an expiration date from the date of purchase unless otherwise stated in writing.
Packages and memberships are non-transferable and non-refundable unless otherwise required by Florida law.

Prior to receiving any services, patients are required to:

    Complete a full medical history and intake form honestly and accurately
    Disclose all current medications, supplements, allergies, and relevant medical conditions
    Inform our team of any changes to your health status or medications prior to each visit
    Follow all pre-treatment and post-treatment instructions provided by our providers
    Report any adverse reactions, side effects, or concerns to our clinical team promptly

Failure to provide accurate and complete health information may affect the safety and effectiveness of your treatment, and Forever Young BHRT will not be liable for adverse outcomes resulting from incomplete or inaccurate disclosures.

Results from any treatment, therapy, or protocol offered at Forever Young BHRT — including hormone therapy, peptide therapy, IV therapy, weight loss programs, and injection therapies — vary between individuals and are not guaranteed.

    We make no representations or warranties that any specific outcome will be achieved.
    Ongoing adherence to your prescribed protocol, follow-up appointments, and lifestyle factors significantly influence results.
    Some therapies may involve off-label use of medications, which will be disclosed and discussed during your consultation.
